Salary Range: $112,484 - $138,984
Duties

Elicits recommendations regarding continuous improvement of quality and patient safety programs from staff and develops recommendations for improvement,

Encourages reporting of occurrences without fear of reprisal.

Investigates all occurrences.

Leads root cause analyses.

Leads the implementation and monitoring of quality, patient safety and performance improvement programs.

Develops and leads Performance Improvement education programs for all staff, including management development programs/ Ensure facility and system-wide learning around Performance Improvement

Supports all departments/units with identifying performance improvement initiatives for each calendar year.

Oversees data collection, chart abstraction and reporting activities as necessary.

Responsible for Quality measures identification and reporting.

Responsible for Quality Assurance and Performance Improvement (QAPI) reports and preparation for monthly Quality Outcomes Committee meetings

Incorporates information learned from tracer and survey activities into Performance Improvement programs

Utilizes system-wide and facility-wide dashboards to track Quality & Safety activities

Liaison to clinical departments identifying and assessing problems and monitoring actions.
Requirements
-
Valid nursing license (RN) in good standing with state licensing board.

Proven experience in hospice & palliative medicine or related fields such as critical care, ICU, emergency medicine, or medical-surgical units.

Minimum of five (5) years’ experience in Nursing and minimum of two (2) years of experience in Quality Assessment, Patient Safety or Performance Improvement, required.

Knowledge of and experience with data, analysis and reporting.

Experience with EMR/EHR systems.

Excellent communication skills for triage calls, family education sessions, case management coordination, and interdisciplinary collaboration.

Knowledge of infection control practices and aseptic techniques essential for safe patient care delivery.

Ability to work independently in home settings while maintaining high standards of safety and professionalism.
