Job Description
Carease Health - NJ is seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team in Tinton Falls, NJ. As a Social Worker/Care Companion, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.
About the Role
Our innovative approach to care combines Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to prioritize proactive measures for enhanced physical health and well-being. As a key member of our team, you will be responsible for:
Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs
Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns
Documenting visit notes and maintaining detailed records of interactions and interventions after each visit
Collaborating with psychiatric professionals to ensure comprehensive care
Participating in a monthly meeting with the psychiatric NP
Responsibilities
Visit designated facilities in accordance with the schedule set with your supervisor
Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
Document visit notes and maintain detailed records of interactions and interventions after each visit
Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
Qualifications
Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field
Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
Strong communication, documentation, and relationship-building skills
Education: Associate Degree
Experience: 1 year
