WAGE

Responsibilities:
The Habilitation Associate will be responsible for providing community based support to individuals based on the needs outlined in the individuals’ ISP. Duties includes, but is not limited to:

Develop and implement goals for assigned individuals

Prepare daily/weekly activity schedules
-
Assist and instruct individuals during community based activities as per their goal plans
-
Collect, maintain, analyze and interpret data on individual’s activities, as required
-
Submit required paperwork within the electronic health record in an accurate and timely manner
-
Provide transportation to and from activities
-
Attend meetings, as required and attend 24 hours of in-service training per in-service year
-
Performs all other duties related to the position as assigned by the supervisor

Demonstrate knowledge regarding fire and emergency evacuation procedures and equipment
-
Understand that added work or irregular hours may be required due to government agency or funding source requests for special assignments
Qualifications:
- Must be at least 18 years of age or older

High School Diploma or GED

Valid driver’s license*

Valid automobile insurance*

Use of personal vehicle*

Computer skills

Act 34 and FBI Clearances (if applicable)
- If non-driving position applicants must have reliable, consistent access to public transport
Minimum Compensation: USD $18.50/Hr.
