Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or transfer belts
- Monitor and record vital signs, medical documentation, and patient observations accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Administer medications and perform catheter care following medical protocols and HIPAA regulations to ensure patient safety and confidentiality
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral management strategies
- Support patients with hospice or palliative needs by delivering compassionate care aligned with their wishes and medical plans
- Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a safe, clean environment in accordance with health standards
- Document all patient interactions thoroughly in medical records while adhering to privacy regulations and ensuring compliance with healthcare policies
Qualifications
- Proven experience working in home health settings, nursing homes, assisted living facilities, or long-term care environments
- Knowledge of medical terminology, medication administration, vital signs monitoring, and dementia or Alzheimer's care techniques
- Certification as a CNA (Certified Nursing Assistant) or Home Health Aide is required; CPR certification is highly preferred
- Experience working with individuals with developmental disabilities or disabilities requiring specialized support is a plus
- Familiarity with EMR/EHR systems for accurate documentation and patient record management
- Ability to lift patients safely using Hoyer lifts or other assistive devices; strong patient observation skills are essential
- Excellent communication skills to collaborate effectively with patients, families, and healthcare teams
