Harmonia Home Health is seeking a highly experienced Registered Nurse (RN) to join our leadership team as an Clinical Director. This is not a training or entry-level opportunity—we are looking for a confident, clinically strong leader with prior experience in a similar role who can contribute immediately.
As Clinical Director, you will support clinical operations, oversee nursing performance, and ensure excellence in patient care, documentation, and compliance. This is a client-facing leadership role serving as a key clinical point of contact for patients, families, physicians, case managers, and field staff.

**Key Responsibilities**
Clinical Oversight & Documentation
- Support clinical workflows within AlayaCare (EHR)
- Review and audit nursing documentation, physician orders, and care plans for accuracy and regulatory compliance
- Conduct both RN and LVN assessments, including in-home client visits as needed
- Ensure all documentation meets HIPAA standards and California home health regulations
Care Coordination & Communication
- Collaborate with physicians and case managers regarding orders, documentation, and care plans
- Serve as a clinical resource for resolving patient care concerns and quality issues
- Partner with administrative staff to support accurate clinical documentation for billing purposes
Leadership & Quality Improvement
- Mentor and support nursing staff in clinical best practices
- Assist in competency validation and ongoing clinical performance oversight
- Monitor and maintain compliance with California home health regulations
- Identify opportunities for process improvement and support implementation of clinical enhancements
Staffing Support
- Assist in coordinating nurse assignments based on patient acuity and care needs
- Provide clinical input on scheduling and coverage decisions to ensure safe, effective staffing
- Serve as a resource to field staff to support appropriate care delivery
Policy Development & Operational Support
- Assist in the development, implementation, review, and ongoing refinement of clinical policies, procedures, and workflows
- Support rollout and staff education related to updated policies and operational processes
- Help establish scalable clinical systems and best practices aligned with company growth
- Identify operational gaps and collaborate with leadership on process improvement initiatives
- Contribute to maintaining survey readiness and regulatory compliance standards
Qualifications
- Active Registered Nurse (RN) license – State of California (required)
- Minimum 5 years of nursing experience and 2+ years in a leadership role
- Prior experience in home health, case management, or clinical operations strongly preferred
- Demonstrated success in a **hands-on clinical leadership role (ADON, Clinical Supervisor, or equivalent)**
- Strong knowledge of patient care standards, regulatory compliance, and documentation requirements
- Experience with EHR systems
- Excellent clinical judgment, communication, and leadership skills
- Detail-oriented with strong critical thinking and problem-solving ability
- Experience developing, implementing, or updating clinical policies and procedures preferred
- Prior experience working within a startup, rapidly growing agency, or evolving operational environment strongly preferred
- Demonstrated adaptability and ability to function effectively in dynamic environments with evolving processes and priorities
**Additional Information**
- This position is on-call evenings, weekends, and holidays as needed
- The Clinical Director is expected to remain clinically accessible and responsive to urgent patient and operational needs
