Luxe Home Health is seeking a compassionate and experienced Clinical Team Manager (RN) – In Office with required home health experience to join our growing team.
This in-office clinical coordination role provides oversight and support for home health patients and field clinicians, ensuring high-quality, compliant, and patient-centered care. The Clinical Team Manager collaborates closely with the interdisciplinary team to oversee care delivery, support quality initiatives, and optimize patient outcomes.
Key Responsibilities
- Serve as the primary in-office Clinical Team Manager for assigned home health patients.
- Coordinate care across disciplines to support field clinicians and patient needs.
- Assess, plan, implement, and evaluate individualized care plans for home health patients.
- Provide clinical oversight to ensure quality care delivery and regulatory compliance.
- Ensure adherence to all state, federal, and agency home health regulations.
- Participate in quality assurance reviews and continuous performance improvement initiatives.
- Communicate effectively with patients, families, physicians, and interdisciplinary team members.
- Maintain accurate, timely, and compliant clinical documentation.
Qualifications
- Active Illinois Registered Nurse (RN) license (required).
- Minimum 2 years of home health nursing experience (required).
- Case management or care coordination experience in a home health setting.
- Strong knowledge of home health regulations and documentation standards.
- Excellent communication, organizational, and problem-solving skills.

Pay: $80,000.00 - $85,000.00 per year
