Certified Behavioral Case Manager is responsible for providing advocacy through intake, developing Individual Service Plans (ISP), care coordination, supporting, monitoring goals and outcomes for the clients of Optimum Potential Community Services. CBHCM is required to have at least a Bachelor's degree, become a certified behavioral health case manager, with experience of 2 years.
