JOB DESCRIPTION
POSITION: Staff Nurse
SUPERVISOR: Nurse Manager

- JOB SUMMARY: Provides nursing care to health centers’ patients within the scope of employee’s certification, according to the CAA policies/procedures, and at the direction of the provider staff.
- SPECIFIC DUTIES:
ESSENTIAL FUNCTIONS
- Follows CAA policies/procedures, including current HIPAA regulations and Ohio scope of practice guidelines
- Provides guidance to health center’s clerical and nursing staff as credentials permit
- Provides direct patient care in accordance with evidence-based health center policies/procedures and provider’s orders
- Implements standing orders in accordance with health center policies and procedures
- Assists in planning and developing a work plan to advance medical home principles including but not limited to team huddles and team based care
- Completes patient screening, education, referral, and follow-up appropriate and in accordance with health center protocols
- Assist with care coordination for patients and identifying groups of patients for care services
- Assist with providing reminders for needed services
- Prepares and completes patient charts and other required patient care documentation and reports as required by health center policy/procedure/protocol and as assigned
- Performs telephone triage of health center patients in accordance with health center protocols and/or provider direction
- Maintains computer records regarding patient information, statistical information or other pertinent data
- Organizes care of patients with health care professionals, care team members and families in making decisions about treatment and attaining health goals
- Participates in performance improvement activities, collaborations and committees as assigned
- Cross-trains with other health center staff as assigned
- Actively supports quality improvement efforts, works to improve patient experience, participate with setting clinical goals and support PDSA execution.
- Participates in health fairs and community outreach programs as assigned
14. Trains or assigns team member to train and coordinate care of individual patients and support family members
15. Performs in-house labs; prepares specimens for reference lab.
16. Schedules follow up testing and referrals per provider’s direction.
17. Assist in developing and participating in staff training to include such topics as, but not
limited to, communication/skills/literacy, self management, population management,
behavior modification, care coordination, and team dynamics
- Travel required for trainings, CME opportunities, satellite sites, etc.
18. Completes a minimum of one hour in infection control training per year
19. Maintains compliance with all laws, regulations and policies as defined by Federal, State
and any other regulatory body
20. Other duties as assigned
- REQUIRED JOB QUALIFICATIONS:
- Graduate of accredited baccalaureate, associate or diploma nursing school
- Current Ohio license as RN or LPN (If LPN, must possess current pharmacology certificate)
- Current CPR certification
- Annual CMEs as required to maintain licensure/certification
- ADDITIONAL REQUIREMENTS AND CONDITIONS OF EMPLOYMENT:
- 1-2 years of experience in health care is preferred
- Effective interpersonal communication skills
- Knowledge of normal development and prenatal health
- Ability to identify and counsel at-risk patients
- Ability to elicit patient participation and understanding
- Background check including Ohio Bureau of Criminal Investigation, Medicare exclusion list, verification of valid driver’s license, employment medical exam, including a TB test, adherence to all CAA policies/procedures and requirements of all funding sources. CAA is a drug/alcohol-free and smoke-free workplace and post-offer drug and alcohol testing is required.

- Ability to accurately carry out orders of providers in a timely manner
- Initiative to understand and carry out tasks necessary to assure good patient care without constant supervision
- Case management and other services according to contract provisions for outside clientele (i.e., Head Start, occupational medicine clients, etc.)
- Demonstrates nursing skills (vitals, labs, etc.)
- 90-day probationary period
- PHYSICAL REQUIREMENTS:
- Ability to lift/carry up to 50 pounds
- Ability to push, pull, carry, lift, bend, twist, reach, sit, stand for lengthy periods of time; ability to walk for extended periods of time
- Ability to work in a fast-paced and often high-stress environment as well as the ability to adapt quickly to workload changes
- Ability to deal with inside and outside clientele in a diplomatic and courteous manner while assuring adherence to CAA policies/procedures
- Above-average oral, reading and writing skills to accurately operate a computer, perform necessary documentation and effectively communicate with staff, patients, clients, customers, business partners, other CAA departments and visitors
WORKING CONDITIONS:
- Ability to remain calm and cheerful under stressful working conditions

- Exposure to chemicals and substances common to an office setting
SALARY: FLSA CLASSIFICATION:
Dependent upon qualifications and experience Hourly, non-exempt
Job Type: Full-time
Pay: $16.00 - $20.00 per hour
