Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health by taking vital signs and observing for changes in condition
- Administer medications following care plans and HIPAA regulations to ensure patient privacy and safety
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Support patients with memory care, dementia, Alzheimer's, and other cognitive challenges through engaging activities and supervision
- Operate specialized equipment such as Hoyer lifts for safe patient transfers and heavy lifting tasks
- Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with healthcare standards
Experience
- Prior experience in home health, assisted living, nursing homes, or group homes is highly desirable
- Knowledge of patient observation techniques and monitoring vital signs accurately
- Hands-on experience working with individuals with developmental disabilities, autism, or dementia-related conditions
- Familiarity with care plans, medication administration, and HIPAA compliance standards
- Experience providing hospice or end-of-life care is a plus
- Ability to perform light housekeeping tasks such as cleaning and maintaining a safe environment for clients
- Strong communication skills for collaborating with healthcare professionals, families, and social workers
