Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, including vital signs and overall condition
- Administer medication reminders and assist with medication administration as directed by healthcare providers
- Support clients with specialized needs such as Alzheimer’s care, dementia care, memory care, and hospice care
- Operate Electronic Health Record (EHR) systems to document medical records accurately and maintain compliance with HIPAA regulations
- Use medical terminology appropriately to record observations and communicate effectively with healthcare teams
- Employ assistive devices such as Hoyer lifts and perform catheter care safely when required
- Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for clients
- Support clients in community outings or errands to promote social engagement and independence
Requirements
- Proven experience in home health care, assisted living, nursing homes or long-term care environments
- Knowledge of medical terminology, vital signs monitoring, medication administration, and documentation procedures
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice or Alzheimer’s care are a plus
- Ability to work with people with disabilities or developmental disabilities with patience and empathy
- Familiarity with EMR (Electronic Medical Records) systems or EHR (Electronic Health Records) systems for accurate record keeping
- Experience working with seniors or in home & community care settings preferred
- Strong communication skills to collaborate effectively with healthcare teams and families
- Physical ability to lift or transfer patients using equipment like Hoyer lifts; capable of performing patient observation duties throughout shifts
- Commitment to maintaining patient privacy and confidentiality in accordance with HIPAA regulations
