Working within the scope of their professional license, the Team Lead develops individualized treatment plans, collaborates with the interdisciplinary care team, assists with department workflow, and helps foster a positive, patient-centered environment that supports quality outcomes and operational efficiency.

Key Responsibilities
· Evaluate residents to determine functional deficits, rehabilitation potential, and individualized treatment needs.
· Develop, implement, and modify individualized treatment plans based on clinical findings and resident goals.
· Provide skilled therapy interventions consistent with evidence-based practice and professional standards.
· Monitor resident progress and modify treatment plans as appropriate.
· Recommend adaptive equipment, assistive devices, and environmental modifications to maximize safety and independence.
· Educate residents, families, and caregivers regarding treatment plans, home exercise programs, safety precautions, and discharge recommendations.
· Participate in discharge planning to support safe transitions of care and reduce rehospitalization risk.
· Serve as a clinical resource for therapy staff regarding patient care, treatment approaches, and best practices.
Team Leadership & Department Coordination
· Assist the Director of Rehabilitation with the daily coordination of rehabilitation services and departmental operations.
· Provide leadership, guidance, and mentorship to therapists, therapist assistants, rehabilitation aides, and students.
· Support onboarding, orientation, and ongoing training of new rehabilitation staff.
· Promote collaboration, accountability, and professional development within the rehabilitation team.
· Assist with scheduling, caseload distribution, and workload balancing to ensure efficient delivery of therapy services.
· Monitor daily workflow and communicate staffing or operational concerns to the Director of Rehabilitation.
· Help ensure continuity of patient care during staff absences or schedule changes.
· Assist with resolving day-to-day clinical and operational issues while maintaining positive team relationships.
Documentation & Regulatory Compliance
· Complete evaluations, treatment notes, progress reports, recertifications, discharge summaries, and required documentation accurately and within established timeframes.
· Ensure documentation supports medical necessity, skilled services, and reimbursement requirements.
· Monitor documentation quality and provide coaching to team members regarding documentation standards and regulatory compliance.
· Maintain compliance with Medicare, Medicaid, PDPM, CMS regulations, state practice acts, HIPAA, and facility policies.
· Participate in MDS processes, care planning, and interdisciplinary documentation as required.
· Assist with maintaining department readiness for audits and regulatory surveys.
Interdisciplinary Collaboration
· Collaborate with physicians, nursing staff, rehabilitation assistants, social services, dietary, and other interdisciplinary team members to coordinate comprehensive resident care.
· Participate in interdisciplinary meetings, care conferences, and family meetings.
· Communicate resident progress, treatment recommendations, discharge planning, and operational updates effectively with the healthcare team.
· Promote effective communication between rehabilitation staff and facility leadership.
Quality & Performance
· Maintain productivity standards established by the rehabilitation department while supporting department-wide productivity goals.
· Assist with monitoring key quality indicators, patient outcomes, and departmental performance metrics.
· Participate in quality improvement and performance improvement initiatives.
· Support infection prevention, fall prevention, resident safety, and risk management programs.
Additional Duties
· Serve as the acting rehabilitation department lead in the absence of the Director of Rehabilitation, as delegated.
· Perform other duties as assigned by the Director of Rehabilitation, Facility Administrator, or designee.
· Maintain regular attendance and adhere to all facility policies, procedures, and professional standards.
