Perform functional mobility, balance, and strength assessments
Develop and implement individualized, goal-driven treatment plans
Provide skilled PT interventions to improve mobility, safety, and independence
Educate residents and caregivers on mobility strategies and fall prevention
Recommend appropriate assistive devices and environmental modifications
Collaborate with an interdisciplinary care team
Complete timely and accurate clinical documentation
Graduate of an accredited Physical Therapy program
Current or eligible Wisconsin PT license
Strong clinical reasoning and communication skills
Passion for working with older adults with dignity and compassion
Flexibility and a collaborative mindset
