Job Summary:
The Licensed Practical Nurse (LPN) for the Group Adult Foster Care (GAFC) and Adult Foster Care (AFC) programs provides direct nursing support and assists in the coordination of care for members in community-based settings. Working under the supervision of a Registered Nurse (RN), the LPN helps ensure members receive safe, consistent, and person-centered care while maintaining compliance with all applicable state regulations.
Key Responsibilities:

Assist the RN with initial and ongoing clinical assessments of GAFC/AFC members

Support the development and implementation of individualized care plans, including Activities of Daily Living (ADLs)

Conduct routine in-home or site-based visits to monitor member health and safety

Provide direct nursing care as appropriate, including basic treatments and health monitoring

Observe and report changes in member condition to the RN or appropriate healthcare provider

Educate caregivers and direct care staff on basic health care tasks, infection control, and safety practices

Assist with medication oversight and reinforce proper medication administration procedures (as permitted by scope of practice)

Maintain accurate and timely documentation, including visit notes and incident reports

Collaborate with case managers, caregivers, and interdisciplinary team members

Attend and actively participate in bi-monthly team meetings for care coordination and case discussion

Report suspected abuse, neglect, or exploitation in accordance with guidelines from the Massachusetts Executive Office of Elder Affairs and the Disabled Persons Protection Commission

Respond to routine clinical concerns and escalate urgent issues to the RN as needed
Qualifications:

Active Licensed Practical Nurse (LPN) license in the Commonwealth of Massachusetts (required)

Graduate of an accredited practical nursing program

Minimum of 1 year of nursing experience; home care, long-term care, or community health preferred

Experience with GAFC/AFC or similar community-based programs preferred

Valid Massachusetts driver’s license and reliable transportation required
