Job Description:
We are seeking a committed Physical Therapist to join a skilled nursing and post-acute rehabilitation center in Sacramento, CA. The facility serves adults and seniors recovering from hospitalization, surgery, and complex medical conditions. You will conduct evaluations, develop evidence-based treatment plans, and provide interventions focused on gait, balance, strength, and functional independence. Responsibilities
Perform comprehensive physical therapy evaluations in a SNF/post-acute setting.
Develop and implement individualized treatment plans based on patient goals.
Deliver therapeutic exercises, gait training, and balance retraining.
Reassess patients and modify plans of care as conditions change.
Educate patients and caregivers on safe mobility and home programs.
Document all care in accordance with regulatory and facility standards.
Collaborate with nursing, physicians, and other disciplines to coordinate care.
Experience / Qualifications
Valid state license as a Physical Therapist.
SNF, inpatient rehab, or post-acute experience preferred.
Strong clinical assessment and treatment-planning skills.
Ability to manage a varied caseload and prioritize effectively.
