Job Summary
We are seeking a dedicated and highly skilled MTM (Medication Therapy Management) Pharmacist to join our dynamic healthcare team. Duties
- Conduct thorough medication therapy reviews, including patient assessments and medication reconciliation to identify potential drug interactions, contraindications, and adherence issues.
- Develop personalized care plans in collaboration with healthcare providers to optimize medication efficacy and safety.
- Manage compounding medications with precision, ensuring adherence to USP 797 standards for sterile preparations and aseptic techniques.
- Provide expert guidance on pharmaceutical regulatory compliance, including state and federal pharmacy laws, drug regulation policies, and HIPAA confidentiality policies.
- Educate patients on medication administration, proper usage, potential side effects, and immunizations to promote health literacy and patient engagement.
- Utilize pharmacy software systems such as Pharmacy Information Systems (PIS), Electronic Health Record (EHR) systems, and EMR platforms for accurate documentation and medication management.
- Oversee inventory management processes, including stock control of medications used in acute care settings such as IV infusions and pain management protocols.
- Lead pharmacy teams by managing daily operations, ensuring adherence to healthcare compliance standards, and fostering a culture of continuous improvement.
- Maintain proficiency in clinical pharmacy practices across diverse settings including community pharmacies, LTC (Long-Term Care), hospital environments, pediatrics, and critical care units.
- Ensure all pharmacy operations align with current drug regulation standards while practicing within scope of practice as a licensed pharmacist.
Experience
- Prior experience in clinical pharmacy roles with a focus on medication therapy management or medical management is essential.
- Demonstrated expertise in managing pharmacy teams within LTC or hospital settings is highly desirable.
- Critical care experience or acute care background enhances your ability to handle complex patient cases effectively.
- Strong knowledge of physiology, anatomy, pharmacology principles, and medical terminology supports accurate patient assessment and medication management.
- Proven ability to work with pharmacy software systems such as PIS, EMR/EHR platforms, and inventory management tools is required.
- Experience with compounding medications using aseptic techniques in compliance with USP 797 standards is preferred.
- Certifications in immunizations (e.g., Basic Life Support) and practicing as a pharmacist under state and federal laws are necessary qualifications.
- Excellent customer service skills combined with effective client communication are vital for engaging patients and healthcare teams alike.

Pay: $60.00 - $67.00 per hour
