Home Health Registered Nurse (RN) Case Manager
$100,000-$103,000/year (including bonuses) + Mileage Reimbursement

Qualifications
- Active Registered Nurse (RN) license in California
- Minimum (2) year nursing experience required as a RN (Required)
- Prior home health experience preferred.
- 2+ year of Manager experience (Preferred)
- Comfortable caring for pediatric and adult patients
- Current BLS certification (Hands-on)
- Strong communication, organization, and clinical judgment
Position Overview
As an RN Case Manager, you will manage and coordinate patient care while collaborating closely with physicians, caregivers, and interdisciplinary teams. You’ll spend your time in patient homes and the field, ensuring care plans are effective, compliant, and patient-centered.
RN Case Manager Responsibilities
- Develop, implement, and revise individualized plans of care
- Complete admissions, re-certifications, and discharges
- Coordinate care with physicians and interdisciplinary team members
- Educate patients and families on care needs and treatment plans
- Supervise and support nursing staff
- Monitor patient progress and communicate changes in condition
- Ensure timely, accurate clinical documentation
- Participate in care conferences and staff meetings
