Job Overview
We are seeking a dedicated and experienced Case Manager Registered Nurse (RN) to join our healthcare team. Duties
- Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to inform care plans.
- Develop and implement individualized case management strategies that encompass discharge planning, home health coordination, and post-acute care arrangements.
- Collaborate effectively with healthcare professionals across disciplines—including physicians, therapists, social workers, and hospital staff—to coordinate comprehensive patient care.
- Manage utilization review processes to ensure appropriate resource use while complying with healthcare policies and managed care guidelines.
- Maintain accurate electronic health records (EHR) and medical records in accordance with HIPAA regulations and hospital standards.
- Facilitate diagnostic evaluations and coordinate services for patients in various settings such as inpatient units, outpatient clinics, or emergency departments.
- Provide patient education on health conditions, treatment options, medication management, and self-care strategies.
- Oversee intake procedures for new patients requiring complex case management or specialized services such as infectious disease care or trauma support.
- Conduct ongoing patient monitoring by reviewing vital signs and clinical data to adjust care plans proactively.
- Assist in discharge planning by coordinating follow-up appointments, home health services, hospice care when appropriate, and ensuring continuity of care.
- Engage in clinical case management activities within diverse environments including ICU, PICU (Pediatric Intensive Care Unit), OR (Operating Room), or behavioral health units.
Experience
- Proven nursing experience with a minimum of three years in hospital settings; critical care experience preferred.
- Demonstrated expertise in Level I or Level II trauma center environments; ICU experience highly desirable.
- Familiarity with emergency room procedures and acute care protocols across various specialties such as medical-surgical, pediatrics, geriatrics, and behavioral health.
- Strong knowledge of hospital medicine practices along with outpatient clinic experience.
- Experience managing complex cases involving infectious diseases or post-acute recovery processes.
- Proficiency in electronic health records (EHR/EHR systems), medical terminology, and healthcare information technology tools.
- Understanding of health regulation policies including HIPAA compliance and medical records management.
- Excellent patient service skills with the ability to communicate effectively across diverse populations.
- Ability to perform detailed patient assessments and manage multiple cases efficiently within a fast-paced environment.
- Knowledge of discharge planning procedures and utilization management processes to optimize patient outcomes.
