Advanced Practice House Calls – Nurse Practitioner or Physician Assistant for Home Visits
Position: Part-Time W2 Role with Benefits (3 fixed days: 9 AM–5 PM)
Location: North Houston and Surrounding Areas (e.g., The Woodlands, Spring, Tomball)
This position involves in-home medical visits for evaluation and management. No procedures required.

Compensation & Benefits (Part-Time – 3 Fixed Days Weekly)
- Annual Salary: $60,000–$70,000 (based on experience)
- Biweekly Pay: $2,308–$2,692 (6 working days per pay period) + $75
- Transportation Stipend: $75 per pay period
- Sign-On Bonus: $700 after 6 months with 1-year commitment
- Performance Bonuses: Based on documentation and productivity
Paid Time Off:
- 6 paid holidays annually (if they fall on a scheduled workday)
- 4 PTO/sick days per year (accrued monthly; available after 6 months)

Position Details
- Schedule: 3 fixed weekdays, 9 AM–5 PM (no weekends, holidays, or on-call)
- Patient Load: 6–7 patients/day (mostly follow-ups)
- Office Time: 2 hours paid; patient load reduced on those days
- Visit Duration: 25–60 minutes
- EHR: Practice Fusion
- Driving: Optimized routes; daily drive time typically under 3 hours
- Routing/Scheduling: Managed by your assigned MA
Responsibilities
- Diagnose and manage acute, chronic, and episodic conditions
- Prescribe medications and treatments
- Perform physical exams
- Create and manage patient care plans
- Coordinate care with home health agencies
Supervise and adjust HH services as needed

Your assigned Medical Assistant handles:
- Route planning and confirmations
- Calls and messages
- Faxing and processing orders
You focus on care — we handle the rest.
Preferred Skills and Experience
- 1 year of NP/PA experience (preferred, not required)
- New grads are strongly encouraged to apply — ideal for those eager to grow in a supportive environment
- Must have a PECOS-registered Medicare number
- Spanish-speaking candidates are highly desirable
- Background in home health is a plus
