Position Summary
Provides physical therapy services to infants and toddlers enrolled in the Connecticut Birth to Three program through community-based and natural environment interventions within home and community based settings. Conducts comprehensive developmental evaluations and collaborates with families, educators, and multidisciplinary teams to support children's participation in educational activities, daily routines, and successful transition to preschool special education services. As a member of an experienced team comprised of PT, OT, SLP, LCSW, Special Ed. you will support children and their families in achieving their outcomes.
Job Responsibilities
- Conduct physical therapy evaluations and ongoing assessments for infants and toddlers referred to the Connecticut Birth to Three program.
- Develop, implement, and monitor individualized intervention plans through the Individualized Family Service Plan (IFSP) process.
- Provide physical therapy services within home and community based settingsusing evidence-based, family-centered intervention strategies.
- Collaborate with teachers, early childhood staff, families, and multidisciplinary team members to integrate therapeutic strategies into classroom routines and daily activities.
- Coach educators, caregivers, and families on intervention techniques that promote functional mobility, motor development, participation, and independence across educational and community settings.
- Participate in multidisciplinary and transdisciplinary team meetings to develop, implement, and evaluate individualized intervention plans.
- Serve as a Service Coordinator when assigned, collaborating with families and community providers to coordinate services and supports.
- Consult with local school district personnel to support children's participation in preschool programs and facilitate successful transitions from Birth to Three services to preschool special education.
- Prepare timely evaluations, progress reports, IFSP documentation, service logs, and other required records in accordance with Connecticut Birth to Three, Connecticut State Department of Education, and agency requirements.
- Maintain current knowledge of evidence-based pediatric physical therapy practices, early intervention regulations, and educational service delivery models.
- Perform other duties as assigned in support of the Birth to Three program.
Qualifications
- Current Connecticut Physical Therapist license in good standing.
- Experience working with infants, toddlers, and families preferred.
- Experience in early intervention, preschool, or school-based pediatric practice preferred.
- Knowledge of Connecticut Birth to Three, IFSP development, family-centered coaching, and transdisciplinary service delivery preferred.
- Strong communication, collaboration, organizational, and documentation skills.
- Valid driver's license, reliable transportation, and ability to travel between preschool and school district locations.
