Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility
- Monitor and observe patient health status, including vital signs and overall condition
- Administer medications following prescribed instructions and document medical records accurately using Electronic Health Record (EHR) systems
- Support patients with specialized care needs including Alzheimer's care, dementia care, hospice support, and working with individuals with developmental disabilities or disabilities
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively
- Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and medical guidelines
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for the patient
- Document all patient interactions thoroughly while respecting HIPAA regulations to ensure confidentiality and privacy
- Collaborate with healthcare teams to coordinate care plans and update medical documentation regularly
- Provide emotional support and companionship to promote mental well-being
Experience
- Prior experience in home health care or assisted living environments is highly preferred
- Demonstrated ability to work with seniors or individuals with disabilities in long-term care settings
- Experience using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for documentation
- Knowledge of medical terminology relevant to patient care and medication administration
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are advantageous
- Familiarity with medical procedures including vital signs measurement, catheter care, and hospice support techniques
- Previous work experience in nursing homes or assisted living facilities is beneficial but not mandatory—training will be provided for dedicated candidates eager to learn
