Schedule Options Available
- 7am to 3 pm, PT or FT
- 3pm to 11pm, PT or FT
- Weekend Doubles or Singles
Key Responsibilities
- Provide direct nursing care to residents in accordance with care plans
- Administer medications and treatments accurately and safely
- Monitor resident conditions and report changes to RN or physician
- Document care in a timely and compliant manner
- Collaborate with interdisciplinary teams to ensure quality outcomes
Requirements
- At least 1 year of previous Nursing Home/LTC experience as an LPN.
- Active and unrestricted New York Licensed Practical Nurse (LPN) license.
- Current BLS/CPR certification.
- Ability to administer medications, perform assessments, and document care accurately.
- Strong clinical judgment, communication, and organizational skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Must successfully pass a background check, health screening, and drug screening in accordance with facility requirements.
