Full-Time Supports Planner
Salisbury, MD | $21-$22/hour I Full-time (35 hours/week, Monday–Friday, 8:00 AM–4:30 PM)

Position Summary
The Supports Planner provides case management services to Medicaid participants, with a primary focus on assisting individuals transitioning from institutional settings—such as nursing homes—back into the community. This role is responsible for coordinating services across multiple programs and funding sources to ensure participants receive the support necessary to live safely and independently.
Essential Duties and Responsibilities:
- Provide case management services to clients referred by the Maryland Department of Health (MDH).
- Conduct comprehensive assessments to determine client needs for services and supports.
- Empower clients to select service providers and, when appropriate, self-direct their services.
- Utilize the MDH LTSS database system to develop and manage Plans of Service (POS) in compliance with MDH regulations.
- Ensure all policies, procedures, timelines, and mandates established by MDH are followed for Support Planning and POS management.
- Maintain consistent client contact:
- Monthly phone contact
- Quarterly and annual face-to-face visits
- Demonstrate cultural competence and sensitivity to clients’ ethnic, religious, racial, disability-related, and environmental backgrounds.
- Coordinate and facilitate meetings with clients, family members, authorized representatives, and service providers.
- Collaborate with the Supports Planner Manager to review caseloads and receive guidance.
- Participate in trainings, conferences, and webinars to maintain professional development.
- Meet productivity expectations, including a minimum billing requirement of 60% of hours worked.
- Maintain strict compliance with HIPAA and confidentiality regulations.
Minimum Qualifications
- Bachelor’s degree in psychology, social work, sociology, nursing, counseling, or a related human services field.
- Preferred experience in case management, especially with older adults and/or individuals with chronic health conditions or disabilities.
- Valid driver’s license required
- CPR and First Aid certification required
- Proficiency with standard office computers, software, and related systems
- Successful completion of a criminal background check in accordance with MAC guidelines
Knowledge, Skills, and Abilities:
- Strong understanding of Medicaid programs and community-based services
- Knowledge of case management principles and best practices
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and effectively manage a caseload
- Demonstrated cultural competence and ability to work with diverse populations
Physical Demands & Work Environment
- Combination of office and field work, including frequent travel to participant homes, nursing facilities, and assisted living settings
- Ability to sit, stand, and use a computer for extended periods
- Exposure to varied home and facility environments
- Occasional lifting of materials or equipment (up to 25 lbs)
- Flexible schedule may be required to meet participant needs
