Overview
We are seeking a dedicated and compassionate Home Health Aide & Caregiver to join our team. In this role, you will deliver essential in-home care services to clients with diverse needs, including those requiring memory care, hospice support, and assistance with activities of daily living (ADLs). The ideal candidate will possess a strong commitment to patient well-being, excellent observational skills, and the ability to work effectively within home healthcare regulations. Responsibilities
- Provide personal care and assistance with activities of daily living such as bathing, dressing, grooming, and mobility support.
- Monitor and document patient conditions regularly, including vital signs and changes in health status.
- Assist with medication administration following prescribed protocols while maintaining compliance with HIPAA and home health regulations.
- Support patients with specialized needs such as Alzheimer's care, dementia care, and working with individuals with developmental disabilities.
- Operate medical equipment including Hoyer lifts, catheters, and EMR (Electronic Medical Records) systems for accurate documentation.
- Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to promote a safe living environment.
- Collaborate with healthcare professionals to ensure comprehensive patient care plans are followed effectively.
- Maintain patient safety through adherence to first aid, basic life support (BLS), CPR certification, and end-of-life care procedures when necessary.
- Support patients during hospice or palliative care phases, respecting their dignity and comfort.
- Engage in ongoing training related to home health practices, medical terminology, and caregiving techniques.
Qualifications
- Proven experience as a Home Health Aide or Caregiver, preferably with assisted living or nursing home backgrounds.
- Demonstrated knowledge of patient observation, medication administration, and medical documentation.
- Experience working with geriatric patients and individuals with disabilities or cognitive impairments such as Alzheimer's disease.
- Familiarity with EMR/EHR systems and home health agency protocols.
- Ability to perform heavy lifting safely and operate mobility aids like Hoyer lifts.
- Strong understanding of home & community care standards, including hospice and end-of-life support.
- Excellent communication skills coupled with compassion and patience.
- Valid certification in CPR/BLS; additional certifications in hospice or dementia care are advantageous.
- Knowledge of medical terminology relevant to home healthcare settings.
- Ability to work independently while maintaining high standards of patient confidentiality under HIPAA regulations.
