Job Summary:
Homecare Supervisor will coordinate participant care with referral source, participant, participant’s family and/or authorized representative, and Homecare Aide, ensuring quality service delivery.
Responsibilities:
· Documenting participant contacts and activities related to participant services in the participant's file on a mandatory monthly basis and as issues occur.
· Assuring that schedules are created based on the days and hours approved by the ccu; if not, proper paperwork is received from ccu.
· Monitoring homecare aides’ clock ins and clock outs ensuring EVV compliance; Retrieve timesheets as needed for audit and payroll purposes
· Conduct mandatory wellbeing/health monitoring calls with clients as well as homecare to build a strong level of communication regarding client’s health, concerns and overall service.
· Responsible for scheduling and documenting contact and quarterly conferences with each assigned homecare aide.
· Consistently update ccu with all participant’s status and health changes via verbal communication, email and critical event reports
· Assure that all hca completes quarterly In-services in order to stay in compliance.
· Evaluate each assigned homecare aide annually;
· Initiating and/or participating in participant staffing discussions with participant, Homecare Aide, Care Coordinators and Executive Director of Home Care services
Skills:
The ideal candidate should possess the following skills:
- Experience in social work, crisis intervention, and behavioral health
- Proficiency in mentoring, workforce development, child welfare, and childcare
- Strong supervisory skills to guide and support individuals effectively
- Knowledge of motivational interviewing techniques for behavior change
- Background in addiction counseling to assist clients dealing with substance abuse
