Join Our Rehabilitation Team!
We are seeking a compassionate, motivated, and patient-focused Physical Therapist (PT) to join our interdisciplinary rehabilitation team. Our skilled nursing facility provides short-term rehabilitation, long-term care, and post-acute services with the goal of maximizing each resident's independence, mobility, and quality of life.
The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions while collaborating with physicians, nursing staff, occupational therapists, speech-language pathologists, and families to achieve optimal patient outcomes.
Essential Job Responsibilities
- Perform comprehensive physical therapy evaluations and establish individualized plans of care.
- Develop measurable functional goals based on resident needs and physician orders.
- Provide skilled therapeutic interventions including therapeutic exercise, gait training, balance training, transfer training, neuromuscular re-education, manual therapy, pain management, and functional mobility training.
- Evaluate and recommend adaptive equipment, mobility devices, and fall prevention strategies.
- Monitor resident progress and modify treatment plans as clinically appropriate.
- Complete timely, accurate documentation in the electronic medical record in accordance with Medicare, Medicaid, managed care, and facility requirements.
- Participate in interdisciplinary care plan meetings, discharge planning, and family education.
- Collaborate with rehabilitation staff, nursing, physicians, and other healthcare professionals to ensure coordinated resident care.
- Educate residents, families, and caregivers on home exercise programs and safe mobility techniques.
- Maintain compliance with all federal, state, facility, and rehabilitation regulations.
- Assist with quality improvement initiatives and maintain productivity standards established by the rehabilitation department.
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited Physical Therapy program.
- Current Ohio Physical Therapist license in good standing.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work independently and as part of an interdisciplinary team.
- Experience in skilled nursing, post-acute rehabilitation, geriatric care, or long-term care preferred but new graduates are encouraged to apply.
Preferred Experience
- Skilled Nursing Facility (SNF) experience.
- Knowledge of Medicare Part A, Part B, Medicaid, Managed Care, and PDPM documentation.
- Experience using electronic medical records (EMR).
- Knowledge of fall prevention, orthopedic, neurological, and cardiopulmonary rehabilitation.
Physical Requirements
- Ability to stand, walk, bend, kneel, and assist residents throughout the workday.
- Ability to safely transfer, lift, position, and assist residents using appropriate body mechanics and mechanical lifting equipment.
- Ability to lift up to 50 pounds with assistance when necessary.
