The Role: Care Coordinator
As a Care Coordinator, you’ll have the opportunity to make a lasting difference in the lives of children and families.
You’ll partner with children, families, providers, and natural supports to help identify strengths, establish meaningful goals, and connect families with the resources and services they need to thrive. We do this by:
- Building trusting relationships with children and families
- Identifying strengths, needs, and goals
- Coordinating services and supports
- Facilitating Child and Family Team meetings
- Collaborating with providers and community partners
- Helping families navigate systems and access resources
- Working across a variety of settings, including the office, homes, and the community
Every day will look a little different—and every day brings an opportunity to help someone move forward.
What You’ll Need
- Bachelor’s degree
- Valid driver’s license and reliable transportation
- A genuine desire to support children and families
- Strong communication, organization, and relationship-building skills
