Licensed Practical Nurse (LPN) – Private Duty Home Health Care

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) with experience in private duty home health care to provide dedicated one-on-one care for a full-time client in a supportive home environment.
This position offers consistent daytime hours, full-time employment, weekends off, competitive compensation, and the opportunity to make a meaningful impact through personalized patient care.
The client currently utilizes a ventilator and G-tube, with an active care plan focused on progressing toward weaning from both. The ideal candidate will have experience managing complex medical needs and be comfortable providing attentive, skilled nursing care while supporting the client’s goals and quality of life.
Responsibilities Include:
- Providing skilled nursing care in a private duty home setting
- Monitoring and managing ventilator and G-tube needs
- Administering medications and treatments as prescribed
- Performing assessments and documenting patient care accurately
- Collaborating with healthcare providers and family members to support the care plan
- Promoting comfort, safety, and independence for the client
Qualifications:
- Current LPN license in good standing
- Prior private duty home health care experience preferred
- Experience with ventilator care and G-tube management strongly preferred
- Strong clinical skills and attention to detail
- Compassionate, reliable, and dedicated to providing exceptional patient care
