Summary
We are seeking a compassionate and skilled Nurse to join our team at Candlewood Healthcare & Rehabilitation. In this role, you will play a vital part in delivering high-quality patient care within our communities, contributing to our mission of providing loving care and quality service.
This position comes with a starting negotiable salary of $65,000.00 annually
The typical schedule for this opportunity is Monday through Friday from 9am to 5pm. Start and end times are flexible based on community need. This is a hybrid position working onsite at the community Three (3) days a week and Two (2) days a week remote at a home office. There are no On-Call responsibilities associated with this position.
Responsibilities

Completed assessments and treatment planning for maximum reimbursement.

Document medical information accurately and maintain patient records in compliance with regulations.

Collaborate with healthcare team members to develop and implement individualized care plans.

Monitor patients' conditions and report any changes to the appropriate medical staff.

Educate patients and their families about health management and wellness strategies.

Ensure a safe and clean environment for patients by adhering to health and safety protocols.
Requirements

Valid nursing license in the state of Ohio.

Strong knowledge of physiology and patient care practices.

Experience in acute care settings preferred.

Familiarity with medical documentation standards, MDS and ICD-9 coding is a plus.

Excellent communication skills for effective interaction with patients, families, and team members.

Ability to work collaboratively in a fast-paced healthcare environment.
