True Providers, Inc. is hiring compassionate, dependable, and patient Direct Support Professionals (DSPs) to provide one-on-one support for an adult individual living independently in an apartment in Middletown, OH 45044.
This individual requires assistance with all Activities of Daily Living (ADLs) and benefits from caregivers who can provide consistent, structured support while remaining calm during behavioral challenges. Due to the client's behavioral support needs, this position offers an enhanced starting wage of $20.00 per hour.
Job Responsibilities
- Assist with all personal care and Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and meal preparation.
- Provide companionship while encouraging independence and community participation.
- Maintain a safe, clean, and structured living environment.
- Complete daily documentation and follow all individualized support plans.
- Use positive behavior support techniques and de-escalation strategies as trained.
- Transport and accompany the individual to doctor's appointments, medical visits, and other community outings as needed.
- Assist with light housekeeping and other household responsibilities.
Qualifications
- Must be Med 1 Certified (Required).
- Valid driver's license, reliable transportation, and proof of insurance required.
- Experience working with individuals with developmental disabilities and behavioral support needs is preferred.
- Comfortable providing hands-on personal care.
- Excellent communication and documentation skills.
- Must successfully pass all required background checks and complete agency-required training.

House Manager – $22.00/Hour
Location: Middletown, OH 45044
True Providers, Inc. is seeking an experienced House Manager to oversee the daily operations of a residential apartment supporting an adult individual with developmental disabilities and behavioral support needs.
The House Manager serves as the lead staff member for the home, ensuring high-quality care, staff accountability, compliance with agency policies, and person-centered supports. Due to the client's behavioral support needs, this position offers an enhanced starting wage of $22.00 per hour.
Job Responsibilities
- Provide direct support to the individual during scheduled shifts.
- Ensure the client is staffed at all times. In the event of a staff call-off, the House Manager is responsible for finding coverage or personally covering the shift if necessary. This is an essential function of the position.
- Supervise, mentor, and support DSP staff assigned to the home.
- Assist with staff scheduling and maintain adequate staffing coverage for all shifts.
- Ensure documentation is completed accurately and submitted on time.
- Monitor the cleanliness, organization, and safety of the apartment.
- Communicate with agency leadership regarding incidents, staffing concerns, and client needs.
- Coordinate, transport, and accompany the individual to doctor's appointments, medical visits, and community activities.
- Assist with medication administration (when applicable), daily routines, and implementation of support plans.
- Train and mentor newly hired DSP staff.
- Ensure person-centered services are provided while maintaining compliance with agency and state regulations.
Qualifications
- Must be Med 1 Certified (Required).
- Valid driver's license, reliable transportation, and proof of insurance required.
- Previous DSP experience required; supervisory or leadership experience preferred.
- Strong communication, leadership, and organizational skills.
- Ability to effectively manage behavioral situations in a calm and professional manner.
- Must be dependable, flexible, and willing to provide direct coverage when staffing shortages occur.
- Must successfully pass all required background checks and complete agency-required training.
Pay: $22.00 per hour (Enhanced Behavioral Support Rate)
