Job Summary
We are seeking a compassionate and dedicated Hospice Nurse to provide high-quality care to patients with terminal illnesses and their families. The ideal candidate will possess a strong clinical background, excellent communication skills, and a deep commitment to patient-centered care. As a Hospice Nurse, you will play a crucial role in managing pain and symptoms, supporting patients' emotional needs, and ensuring that families receive the guidance and support they need during this challenging time. We focus on the quality and have a great team waiting to meet you. You do not need previous hospice experience as we can train. We service the Denver and surrounding areas. Mountain Ridge Hospice is new to the Denver area and are looking for someone who will help us grow.
We are located in the Colorado Springs however we have a supply location in the Denver area. The nurse will need to come in to the office only monthly for a meeting. We would like to expand in the Denver area and build great relationships so this nurse will play a big role in our growth in the area.
Responsibilities
- Conduct comprehensive patient assessments to determine individual care needs and develop personalized care plans.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols and best practices.
- Collaborate with interdisciplinary teams to coordinate care and communicate effectively with family members about patient status.
- Monitor patients’ conditions, documenting changes in health status, and adjusting care plans as necessary.
- Provide education to patients and families regarding disease processes, pain management techniques, and end-of-life options.
- Ensure compliance with HIPAA regulations while maintaining patient confidentiality at all times.
- Participate in quality improvement initiatives to enhance the delivery of hospice services.
Skills
- Proficient in patient assessment techniques with the ability to recognize changes in condition promptly.
- Strong skills in pain management strategies tailored to individual patient needs.
- Understanding of memory care principles for patients experiencing cognitive decline.
- Ability to engage in family planning discussions when appropriate, providing support and resources.
