Care Manager | Remote | New Mexico | Contract
Walker Healthforce is seeking a
Developer with minimum 2+ years of experience with Statement of Work (SOW) & Vendor contracting. This is a 6-month contract opportunity.
START DATE: 6/29/2026
HOURS/SCHEDULE: 8 - 5pm
WORKER TYPE: Contract
Remote - New Mexico – Las Cruces or Albuquerque.
CORE REQUIREMENTS:
- Minimum of 3 years of experience in behavioral health, substance use disorder treatment, care management, or a related clinical setting
- Active, unrestricted behavioral health clinical license in the state of practice
such as an LCSW/LISW, LPC/LPCC/LMHC, or LMFT only
- Strong working knowledge of behavioral health quality and HEDIS measures, including FUH, FUM, FUI, and IET
ADDITIONAL REQUIREMENTS:
- Behavioral health transitions of care/discharge planning from higher levels of care to lower levels of care (community) or residential settings
- Telephonic outreach following admission into an inpatient facility and structuring a conversation (consider motivational interviewing techniques)
- Unable to engage members and face to face experience
- Coordination with hospitals, providers and other ICT members to prepare for discharge
- Experience with member barriers such as housing, transportation and actions to address.
- Appropriate transitions and follow-up after discharge and identifying high risk behaviors and gaps in care.
- Demonstrated behavioral health care management experience, including transitional support following hospitalizations or emergency department visits
- Strong working knowledge of behavioral health quality and HEDIS measures, including FUH, FUM, FUI, and IET
- Proven ability to conduct telephonic and/or face-to-face outreach with members to support follow-up care
- Experience scheduling and coordinating appointments with behavioral health providers or internal care teams
- Ability to utilize established care management programs to provide ongoing member support
- Excellent clinical documentation skills, with the ability to accurately document member interactions in accordance with job aides and compliance requirements
- Strong communication and collaboration skills, including the ability to work with leadership and interdisciplinary teams
- Demonstrated ability to handle and protect PHI in compliance with healthcare privacy and security standards
- Experience working within secured healthcare systems and accessing clinical platforms remotely
- Ability to work full-time (40 hours/week) during MST business hours (8:00 a.m. – 5:00 p.m.)
- Comfortable working remotely using personal or agency-provided equipment to access virtual desktop environments.
WE CONSIDER IT A BONUS IF YOU ALSO HAVE:
- Experience supporting enterprise-level workforce programs
- Understanding of rate card management and bill rate negotiations
- Experience in healthcare, financial services, or large corporate environments
- Experience with contract management/review
- Bachelor's Degree or equivalent combination of education and experience
JOB RESPONSIBILITIES:
- CM staff responsibilities include:
- Reaching out to members identified for transitional support.
- Scheduling appointments with members’ behavioral health treatment providers or Care Connections.
- Utilizing existing care management programs to provide ongoing member support.
- Documenting all member interactions, ensuring compliance with the job aide to contribute to HEDIS goals.
- Communicating with local leadership regarding any questions or concerns about the procedures.
