Shift: Sun, Mon, Tues, E/O Wed Part Time Days
School Diploma or equivalent. Experience in seeking and accessing state and community resources and a willingness to help families access resources. Experience raising a child with behavioral/emotional issues, accessing services for children with behavioral/emotional issues and navigating the child serving system.

Assist families in developing and recruiting informal supports to be a part of the Family Team meetings.

Complete (or co-complete) Strengths Needs Cultural Discovery (SNCD) assessments, Functional Assessments/Crisis Plans, Safety Plans, and Care Plans/Updates.

Provide support for families with the eventual goal of the family utilizing their natural supports.

Facilitate or co-facilitate Family Team meetings and ensure informal supports are included and part of the team.

Implement the Do For, Do With, and Cheer On model for families.

Maintain close communication with the Integrated Team Manager and other clinical and administrative staff in order to ensure the highest quality services are delivered for youth and families.

Complete flex-fund requests when necessary.

Monitor and evaluate services provided to the families. Contact families as needed to monitor progress and inquire about new needs.

Personality and mature presentation of oneself, demonstrating the ability to work with various systems while maintaining objectivity and cooperative attitude.
