Opportunity Description
Homestead Post Acute is currently looking for a Social Services Director to provide advocacy and support to our long-term care residents.
Responsibilities Include:
1. Completing Social Services Assessments and Social Histories
2. Completing clinical documentation
3. Conducting Resident Care-Plan Meetings
4. Investigating Facility Complaints
5. Working with facility leadership to provide the best care to facility residents.
The Social Services Director's objective is to productively participate in creating an environment where residents psycho-social needs are met.
