El Sol Neighborhood Educational Center is a nonprofit organization in the Inland Empire devoted to empowering our communities to lead healthy lives & access to health care; safe, affordable housing; opportunities for education; and the leadership skills to eliminate disparities. El Sol NEC’s mission is for community members to reach personal empowerment to move towards self-sufficiency and contribute to their community’s well-being.
El Sol Neighborhood Educational Center is seeking a dedicated Community Health Worker to promote health within the community by empowering individuals to adopt healthier behaviors. The CHW will serve as a trusted advocate, ensuring individuals can effectively communicate with healthcare providers and social service agencies. Acting as a liaison, this role supports the implementation of programs that enhance individual and community well-being, including preventive services and data collection to assess community health needs. Outreach efforts will include referrals from clinics and social-service organizations, door-to-door engagement, and participation in health and community fairs.
Minimum requirements:
- High School Diploma or equivalent req; 2-3+ years of related/equivalent experience in a similar position with training/certifications; 80hrs.
- Bilingual (Spanish/ English; speak and write) preferred.
- Receive clearance from an FBI background check (Custodian of Records for DOJ).
Qualifications:
- Minimum of 2 years of experience as a CHW, Promotor de Salud, or similar role.
- Willing to commute within the surrounding work area.
- Strong knowledge of public health principles and community resources.
- Excellent interpersonal, communication, and organizational skills.
- Proficiency in outreach methods, including door-to-door engagement and event participation.
- Basic knowledge of Microsoft Office Suite and data collection tools.
- Ability to work independently and as part of a team.
- A desire for frequent local travel to meet with participants and attend community events.
- Proven ability to multitask, prioritize tasks, and manage time effectively.
- Capable of working/ understanding the needs of a multicultural environment.
- Strong work ethics, maintaining a professional appearance, and exercising discretion with sensitive information.
- Ability to perform tasks related to physical activity to include sitting, standing, for long periods of time, caring, bending, moving, and lifting up to 50 lbs.
Responsibilities:
- Serve as a bridge between the community and healthcare providers, ensuring individuals’ voices are heard and needs are met.
- Advocate for equitable access to healthcare and social services for underserved populations.
- Build trust and foster strong relationships with community members, healthcare organizations, and social service providers.
- Provide culturally relevant health education on topics such as chronic disease prevention, nutrition, mental health and various other facets.
- Assist individuals in developing personalized strategies to adopt and sustain healthy behaviors.
- Educate community members about available healthcare and social resources, guiding them in navigating systems effectively.
- Implement health initiatives that promote, maintain, and improve individual and community health outcomes.
- Conduct door-to-door outreach, attend community fairs, and collaborate with local clinics and organizations to connect with individuals in need.
- Deliver preventive services, such as health screenings and vaccination education, in collaboration with healthcare teams.
- Collect and report data on community health trends, outreach efforts, and program effectiveness.
- Assist in identifying emerging health needs within the community to guide program development.
- Receive and manage referrals from healthcare providers and social-service organizations/partners.
- Facilitate access to medical appointments, social services, and other resources, ensuring follow-through and continuity of care.
- Support and mentor other Community Health Workers, with guidance and sharing best practices.
- Participate in the training and onboarding of new Community Health Workers.
- Help organize and participate in health education workshops, community meetings, and public health campaigns.
- Represent El Sol NEC at health-related events, promoting organizational programs and services.
- Maintain accurate and confidential participant records, ensuring compliance with organizational and regulatory standards.
- Participates in staff meetings, individual supervision, and training related to their work.
- Other duties as assigned.
