Job Overview
We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) for our ADON position join our dynamic healthcare team. In this vital role, you will provide compassionate patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and community clinics. Your expertise will help ensure the delivery of high-quality nursing services, promote patient safety, and support overall well-being. This position offers an exciting opportunity to work with diverse patient populations, including geriatric and developmentally disabled individuals, while utilizing your clinical skills in a fast-paced environment.
Duties
- Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient needs.
- Assist with medical procedures such as catheterizations, venipuncture, and sterile dressing changes following aseptic techniques.
- Manage complex cases involving dementia care, Alzheimer’s disease, memory care, and behavioral health issues in senior or memory care units.
- Document all patient interactions thoroughly using electronic health record (EHR) systems like Epic or Cerner while adhering to HIPAA confidentiality policies.
- Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care strategies to optimize patient outcomes.
- Provide emergency response support including airway management, acute pain management, triage assessments.
- Other duties as requested by the DON
Experience
- Proven experience working in diverse healthcare environments such as Long Term care centers is highly desirable.
- Hands-on experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for medical documentation and records management.
- Strong background in managing complex medical conditions including infectious diseases, chronic pain management, hospice care, and geriatrics.
- Experience working with vulnerable populations such as Geriatric, and those requiring specialized dementia or Alzheimer’s care.
- Ability to perform clinical procedures such as General wound care, Diabetic care

Pay: $30.00 - $34.00 per hour
