Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to develop personalized care plans.
- Administer medications, injections, and catheter care following strict protocols and safety standards.
- Provide patient education on medication management, health coaching, and disease prevention strategies tailored to geriatric patients and those with chronic conditions.
- Collaborate with physicians, case managers, and other healthcare professionals to coordinate discharge planning and post-acute care services.
- Routine monitoring over staff documentation and compliance.
Requirements
- Valid RN or LPN license in good standing; prior experience working in long-term care facilities, skilled nursing facilities, hospitals (including ICU or trauma centers), or home care is highly preferred.
- Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques relevant to geriatrics and complex patient populations.
- Ability to perform clinical procedures such as venipuncture, phlebotomy, sterile processing techniques, specimen collection/processing, and aseptic technique.
- Must take one weekend per month to be on call. For resident emergencies.

Pay: $28.00 - $36.00 per hour
