In this fully remote role, you'll be the lifeline for patients managing chronic conditions like hypertension, heart failure, COPD, and more. You'll monitor real-time vital signs, connect with patients through meaningful conversations, and work with physicians to ensure every patient gets the attention they need — before small issues become big problems.

What you'll do: Monitor patient vitals, conduct wellness check-in calls, provide health education, and escalate concerns to the care team when needed.

Schedule: Monday–Friday 9am - 6pm | No nights | No weekends
