The LPN provides ongoing support and expertise through, planning, implementation and overall evaluation of individual Medicaid beneficiary needs in Madison County
Monday thru Friday, 8am-5pm schedule.

Summary of Qualifications:
- Active LPN with a minimum of two (2) years of human social services, public health, healthcare, or community health experience.
- Possess the knowledge of resources serving Medicaid beneficiaries.
- Experience in demonstrating independent judgement and decision making.
- Have strong verbal and written communication skills with the ability to encourage and engage recipients in plan of care.
- Ability to sufficiently document electronic case records.
- Ability to work independently and collaboratively.
- Computer skills.
- Willingness to travel as required for the job.
- Live in one of the six counties we serve: Madison, Jackson, Marshall, Cullman, Limestone or Morgan.
Primary Responsibilities:
- Assist with auditing and care plan reviews.
- Support maternity case managers with tasks and duties.
- Partner with care team (community, providers, internal staff) to address the needs of Medicaid beneficiaries.
- Other duties as assigned.
