Position Summary
The Clinical Operations Assistant serves as a key member of the clinical leadership support team. This role assists with clinical operations, EMR documentation review, coordination of services, physician communication, quality initiatives, and administrative support for nursing leadership. This position is primarily office-based and ideal for an LPN who enjoys organization, problem-solving, and ensuring high-quality patient care behind the scenes.
Essential Responsibilities
- Support the ADON and DPCS with day-to-day clinical operations.
- Review clinical documentation for completeness and regulatory compliance.
- Assist with physician order tracking and follow-up.
- Monitor unsigned orders and coordinate completion with providers.
- Support hospice admission and recertification processes.
- Assist with preparation for audits, surveys, and quality reviews.
- Review EMR documentation and notify clinicians of missing or incomplete records.
- Coordinate communication between field staff, physicians, and clinical leadership.
- Assist with incident reporting and follow-up documentation.
- Track quality indicators and clinical compliance metrics.
- Provide support with scheduling and staffing coordination as needed.
- Participate in interdisciplinary team coordination.
- Perform other duties as assigned to support clinical operations.
Qualifications
- Current unrestricted LPN license.
- Minimum 2 years clinical nursing experience preferred.
- Hospice, home health, or long-term care experience preferred.
- Strong organizational and time-management skills.
- Excellent communication skills.
- Experience with EMR systems; MatrixCare experience preferred.
- Ability to prioritize multiple tasks and work independently.
- Proficient in Microsoft Office applications.
Preferred Experience
- Hospice experience.
- Quality Assurance and Performance Improvement (QAPI) participation.
- Knowledge of Medicare Conditions of Participation.
- Experience tracking physician orders and clinical documentation.

Pay: $65,000.00 - $75,000.00 per year
