EverHome Home Health Care is seeking a compassionate and dedicated Medical Social Worker (MSW) to join our team on a PRN basis in the Ann Arbor/Brighton areas.
This position offers the opportunity to work directly with patients and families in their homes, helping them navigate healthcare challenges, access community resources, and achieve the highest possible quality of life. What You'll Do:
- Assess patients' social, emotional, financial, and environmental needs
- Provide counseling, support, and guidance to patients and their families
- Connect patients with community resources and support services
- Assist with discharge planning and long-term care planning
- Collaborate with nurses, therapists, physicians, and caregivers as part of an interdisciplinary home health team
- Advocate for patients and help address barriers to care
- Complete timely documentation in accordance with agency and regulatory requirements

Qualifications:
- Master's Degree in Social Work (MSW) required
- Current Michigan Social Work licensure required
- Home health or healthcare experience preferred
- Strong communication, organization, and interpersonal skills
- Reliable transportation and ability to travel within assigned territory
