TITLE: Oncology Nurse Navigator
JOB OVERVIEW: Guides patients through the health care system by assisting with access issues, developing relationships with service providers and tracking interventions and outcome. Promotes a positive teamwork environment which emphasizes respect to all customers, including patients, families, physicians, staff, and other departments. Receives and responds to calls from patients and families for health care assistance using the nursing process in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization. This position will also work collaboratively with clinical care teams in the coordination of care for certain patient symptoms and conditions.

PERFORMANCE RESPONSIBILITIES:
Develops relationships with all oncology providers including but not limited to medical oncologists, radiologist, general surgeon, surgical oncologist, radiation oncologist, pathologist, plastic surgeon, primary care physician, hospital departments and team members. Serves as a liaison between the patient, hospital departments, team members and the providers.
Provides access to community resources and links patients to support services such as: social services, nutrition services, psychologist, pastoral care, physical therapy, rehabilitation therapy, financial counseling, etc.
Coordinate site specific cancer patient multidisciplinary (MDC) visit, document treatment plan summary and disseminate to patient, appropriate departments, and physicians.
Telephone Assessment, Triage, and Care Management:
Performs in-patient assessment, care coordination and professional nursing functions to assess the severity of the patient's health concerns using procedures, protocols and algorithms in a manner that provides patient dispositions to emergency care or clinic provider.
Provides referrals to healthcare providers and community resources when appropriate, including locations, hours, and contact information.
