Job Title: Intake Navigator

We’re hiring a compassionate, organized Intake Navigator to coordinate our Homeless Prevention Program and serve as the first point of contact for individuals seeking shelter or support.
Job Purpose: The Intake Navigator supports the Coordinated Entry process by responding to community members in need of shelter or services, determining eligibility for Home of the Sparrow programs, and providing referrals and resources. The role also includes participation in community outreach events as needed.
Job Overview:
We are seeking a dedicated and compassionate Intake Navigator to join the Home of the Sparrow team. In this important role, you will serve as a primary point of contact for individuals seeking shelter, housing support, and community resources. The Intake Navigator will guide prospective clients through the intake and Coordinated Entry process, assess program eligibility, provide referrals, and connect individuals with appropriate services. This position requires strong communication skills, empathy, and the ability to support individuals experiencing challenging circumstances while maintaining professionalism and confidentiality. Responsibilities
- Respond to community phone calls, emails, and walk-in inquiries while providing referral information and support as needed.
- Develop and maintain knowledge of community resources to provide informed referrals to intake callers and prospective clients.
- Appropriately respond to individuals experiencing crisis situations and provide compassionate, client-centered support.
- Schedule and conduct phone and in-person interviews for prospective clients.
- Complete screenings and eligibility assessments for all Home of the Sparrow programs and coordinate referrals with appropriate case workers.
- Provide referrals, advocacy, and supportive services throughout the intake process.
- Attend community outreach events and engage with local providers as needed.
- Enter client information into HMIS and billing systems and ensure timely updates, including client exits upon discharge.
- Maintain accurate client data, including waitlist and Coordinated Entry documentation.
- Facilitate connections to emergency shelters and other community resources when appropriate.
- Conduct outreach and maintain communication with community partners and service providers.
- Complete monthly reports, quarterly reports, and outcome documentation required by funders.
- Ensure the Intake Procedure Manual remains current and is followed consistently.
- Document client interactions, interventions, and billing entries accurately and in a timely manner.
- Participate in Intake Coordinator meetings with the Mental Health Board and Coordinated Entry Committee meetings through the McHenry County Continuum of Care as needed.
- Meet with the Intake Manager for regular supervision at least monthly.
- Attend and actively participate in team meetings and required agency trainings.
- Represent Home of the Sparrow positively and professionally within the community at all times.
Knowledge & Skills:
- Strong clinical skills preferred.
- Strong written and verbal communication skills.
- Previous experience working with high-barrier populations preferred.
- Good problem solver and strong attention to details.
- Bilingual preferred.
- Ability to work independently, as well as with a team.
- Prior knowledge of McHenry County community resources preferred.
Qualifications:
- Bachelor Degree in Social Work, Human Services or related field is preferred.
- 1-2 years of experience working with high barrier individuals is preferred.
- Has a valid Driver’s License.
Physical Demands:
- Ability to lift up to 30 pounds.
- Balancing, stooping, kneeling, crouching, crawling and reaching are required.
- Sitting, standing and walking for extended periods of time.

Pay: $19.00 - $22.00 per hour
