Youth Consultation Service (YCS) is a behavioral health and social services agency that has been caring for at risk children and families since 1918 in therapeutic residences, special education schools, in-home and in-community programs throughout NJ.
Currently seeking a Case Manager-
Duties include (but not limited to):
- Plan and facilitate psychoeducational groups to support youth development, focusing on areas such as interpersonal effectiveness, emotional regulation, problem-solving skills, goal setting, and conflict resolution
- Maintain ongoing communication and coordination with families and external team members such as DCPP, CMO, CASA, Probation, Law Guardians
- Participate in multidisciplinary team meetings along with clinicians, APNs, CMO’s, DCPP, school staff, probation and others to review youth progress, identify areas in need of improvement, plan/coordinate discharges or transitions to other programs
- Assist with placing youth in the appropriate school setting and advocating for youth within the school system, helping to identify and address any academic and behavioral difficulties
- Provide referral and linkage to ancillary and or specialized treatment services and interventions -Maintain current and accurate documentation within EHR system
Requirements:
Bachelor's with 3 or more years of relevant experience or Master's with 1-year of related experience
