Overview
We are seeking a dedicated and compassionate Psychiatric-Mental Health Nurse Practitioner to join our healthcare team. In this role, you will provide comprehensive mental health care to patients in our outpatient . You will be responsible for assessing, diagnosing, and treating mental health conditions while working closely with patients to develop individualized care plans that promote their overall well-being.
Responsibilities
- Conduct thorough psychiatric evaluations and assessments of patients to determine their mental health needs.
- Develop and implement treatment plans based on evidence-based practices and patient-centered care principles.
- Provide ongoing management of psychiatric medications, including monitoring for efficacy and side effects.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care, including coordination with primary care providers and specialists.
- Educate patients and their families about mental health conditions, treatment options, and coping strategies.
- Maintain accurate and up-to-date patient records in compliance with regulatory standards and best practices.
- Participate in quality improvement initiatives to enhance the delivery of mental health services within the organization.

Qualifications
- Master’s degree in Nursing with a focus on Psychiatric Mental Health
- Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Proficient in patient assessment and care planning
- Experience with EHR system
- Ability to work with diverse populations
