Community Habilitation Worker (903)
Salary: $22.00/hr
Description: Community Habilitation worker positions are available throughout Nassau and Suffolk Counties. Community Habilitation Workers work independently to support individuals with developmental disabilities. Flexible hours are available in the mornings and afternoons throughout the week and weekends. A high school diploma is preferred for all applicants. Candidates use their personal vehicles for transporting individuals into the community: vehicles should be in good working condition.
Shift: Daytime availability preferred (For example: 9:00A – 4:00P)

Assist with building skills in their home as well as providing opportunities for community inclusion.

Qualifications:
- Clean and valid NYS Driver’s License required
- High School Diploma/GED preferred
- Experience working with individuals with developmental disabilities preferred
- Personal vehicle in good working condition
Department Details:
Offering hands on training to I/DD individuals in various skills such as community integration, socialization, independent living. Applicants will need to be able to travel throughout Nassau & Western Suffolk Counties with your personal vehicle. Car and clean NYS Driver License a MUST! Suffolk County Residents are encouraged to apply. There are many individuals in need of this service in Suffolk County!

- Less than 10
- 10-19
- 20-29
- 30-39
Key Responsibilities include, but not limited to:
- Ensure the health and safety of the individual by being observant of person and their environment.
- Develop a strong relationship with the person participating in the program.
- Participate in WCS team meetings and trainings.
- Participate in the development and delivery of the person centered plan.
- Drive consumers to and from recreation outings, or other destinations as indicated in the individual’s plan.
- Work directly with individuals to implement Community Habilitation Plan activities including but not limited to activities of daily living (toileting, feeding, dressing, etc.), independent living skills (laundry, cleaning, money skills, etc.), and community integration skills (recreation, travel training, etc.).
- Document Community Habilitation activities on a daily and monthly basis.
- Maintain contact with families, supervisors and other parties as needed.
- Report situations/issues involving consumers to proper authorities.
- Incorporate Code of Ethics, Core Competencies and Empowerment Strategies in their daily professional responsibilities.
