Specialty Pharmacy 340B Patient Care Coordinator

Previous 340B experience is highly preferred, but we are willing to train an exceptional candidate with specialty pharmacy, hospital outpatient pharmacy, medication access, or prior authorization experience.
Help Patients Access the Care They Need
As an independent, nationally licensed specialty pharmacy serving patients in all 50 states and Washington, D.C., we are seeking a compassionate, service-oriented 340B Patient Care Coordinator to join our growing team. In this role, you will be a trusted advocate for patients, helping them navigate their treatment journey, access specialty medications, and receive the personalized support they need to achieve the best possible outcomes.
Position Summary
The 340B Patient Care Coordinator serves as the primary point of contact for patients, caregivers, healthcare providers, and covered entity partners. This role focuses on creating an exceptional patient experience by coordinating care, providing education and ongoing support, and helping patients successfully access and remain on their prescribed therapies.
Working alongside pharmacists, providers, and internal care teams, the Patient Care Coordinator plays a vital role in improving medication adherence, reducing barriers to care, and supporting the overall success of Perigon Pharmacy 360's 340B program.
What You'll Do
- Serve as the primary point of contact for patients throughout their therapy journey, providing compassionate, personalized support.
- Build trusting relationships with patients, caregivers, healthcare providers, and clinic staff through exceptional customer service.
- Welcome new patients to the pharmacy, explain our services, and guide them through the onboarding process.
- Coordinate prescription fulfillment, refills, and ongoing specialty medication support.
- Educate patients about their medications, treatment expectations, refill schedules, and available pharmacy resources.
- Proactively contact patients to encourage medication adherence, answer questions, and identify barriers that may impact treatment success.
- Collaborate with pharmacists, providers, nurses, and clinic staff to coordinate care and improve patient outcomes.
- Connect patients with financial assistance resources, manufacturer support programs, and community services when appropriate.
- Support patient enrollment and participation within the 340B program while helping maintain program integrity.
- Maintain accurate patient documentation and communicate updates across the healthcare team.
- Protect patient privacy by maintaining HIPAA standards and organizational policies.
- Contribute to a positive, patient-centered culture focused on service excellence and quality care.
What You'll Bring
- Minimum of 2 years of experience working in a 340B pharmacy environment (required).
- Experience in specialty pharmacy, outpatient pharmacy, ambulatory care, or a patient care coordination role preferred.
- A genuine passion for helping patients and delivering outstanding customer service.
- Strong communication and relationship-building skills with the ability to connect with diverse patient populations.
- Ability to educate, encourage, and support patients throughout their treatment journey.
- Strong organizational skills and the ability to manage multiple priorities in a fast-paced environment.
- Experience working with pharmacy management systems and electronic health records.
- Working knowledge of the 340B Drug Pricing Program and specialty pharmacy workflows.
- High school diploma or equivalent required; additional healthcare or pharmacy education is a plus.
