Duties
- Conduct comprehensive patient assessments to evaluate physical, emotional, and spiritual needs, utilizing advanced nursing skills and physiology knowledge.
- Develop and implement individualized care plans that incorporate symptom management, medication administration, IV infusion, pain management, and comfort measures.
- Administer medications, including opioids for symptom control and perform procedures such as suturing or ostomy care as needed.
- Coordinate interdisciplinary team efforts to ensure seamless delivery of hospice services, including discharge planning and case management.
- Educate patients and families about disease progression, medication use (including ICD coding and documentation), advanced directives, and available resources.
- Monitor patient vital signs, perform physical examinations, and utilize EMR systems like Epic or Cerner for accurate medical documentation.
- Provide emotional support and counseling to patients and families while working with individuals with disabilities or dementia care needs.
- Ensure infection control practices are strictly followed to maintain aseptic technique during procedures such as tube feeding or specimen collection.
- Maintain compliance with HIPAA regulations while managing medical records, medical documentation, and patient intake information.
Skills
- Proven experience in hospice & palliative medicine with a strong foundation in acute pain management and symptom control strategies.
- Critical care experience including ICU or Level I Trauma Center background enhances ability to handle complex cases with precision.
- Familiarity with EMR/EHR systems such as Epic or Cerner for efficient documentation and patient tracking.
- Skilled in IV insertion, catheterization (including Foley catheters), wound suturing, ostomy care, and sterile processing techniques.
- Knowledge of ICD-10 coding standards for accurate medical records and billing processes.
- Experience working with diverse populations including geriatrics, pediatrics, individuals with developmental disabilities or memory care needs.
- Ability to perform comprehensive patient assessments through vital signs measurement, physical examinations, laboratory experience, and diagnostic evaluation.
- Strong understanding of infection control protocols, occupational health standards, and emergency procedures like airway management or ventilator support when necessary.
- Excellent communication skills for triage situations, family education sessions, and interdisciplinary collaboration in outpatient or home care settings.
