Duties and Responsibilities
- Develop a person-centered treatment plan that focuses on addressing the needs of the client to include family engagement when applicable

Coordinate care for the client with family and other involved individuals, i.e. school personnel, probation officer, mentor, DHHS, medical personnel, etc

Provide individual, family therapy, and case management, as needed to each client/family on his/her caseload

Market and network with community partners and current/potential referral sources. Examples of this include but are not limited to attending community collaborative meetings, distributing marketing material, providing a description of services provided

Complete all service plans, progress notes, discharge summaries and all other necessary documentation in a timely manner

Represent Samaritas mission and vision with community partners. Exercise excellence and integrity as the job requirements are fulfilled

Monitor the quality of overall treatment services to clients on his/her caseload; and evaluate the adequacy of these services to meet treatment objectives

Participates in regularly scheduled staff meetings, therapist meetings, and any others as deemed necessary by the SUD Leadership

Represent Samaritas mission and vision with community partners. Exercise excellence and integrity as the job requirements are fulfilled

Responsible for timely Quality Assurance compliance as related to employee’s roles and functions
Professional Growth

Possess a working knowledge of licensing and accreditation standards and work to assure compliance with such standards

Utilize educational opportunities and resources offered by the agency and through the profession for continued development of skills and clinical expertise, (conferences, trainings, peer supervision)
Job Qualifications
Education, Training, and Licensure/Certification
- Master Degree in Social Work, Counseling or a related field, from an accredited institution
-
Two years of clinical experience working with SUD clientele preferred
- Prior experience working with an interdisciplinary team preferred.
Experience

Minimum one year SUD counseling experience preferred

Minimum experience working on a team preferred

Familiarity with SUD documentation preferred

Fully licensed clinician preferred. Limited license will be considered
Additional Work Requirements

Valid unrestricted driver's license with good driving record and proof of automobile insurance.
