Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote dignity and independence
- Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals
- Support medication reminder
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately document patient care activities and medical records
- Prepare nutritious meals and assist with light housekeeping tasks like laundry and cleaning to ensure a safe living environment
- Support patients during transportation for medical appointments or community outings while maintaining safety protocols
Requirements
- Proven experience in home health care, assisted living, nursing homes, or long-term care environments
- Knowledge of medical terminology
- Certification in CPR (Cardiopulmonary Resuscitation) is required.
- Ability to operate EMR/EHR systems accurately and maintain strict confidentiality in accordance with HIPAA standards
- Strong communication skills to collaborate effectively with healthcare teams and families
- Physical stamina to perform caregiving tasks that may involve lifting or assisting patients with mobility needs
- Valid driver’s license and reliable transportation preferred for home visits
