Job Summary
The Social Service Designee plays a vital role in connecting individuals and families to essential social services, ensuring they receive comprehensive support tailored to their unique needs. This energetic and dedicated professional will coordinate with community resources, provide crisis intervention, and advocate for clients across diverse populations, including seniors, individuals with disabilities, and culturally diverse communities. Duties
- Conduct thorough client assessments to identify needs related to housing, healthcare, mental health, addiction recovery, and social welfare programs such as Medicaid, Medicare, Child Welfare, and Section 8 housing.
- Develop individualized service plans by collaborating with clients and community partners to facilitate access to resources like behavioral health services, childcare, workforce development programs, and disability support.
- Provide crisis intervention and crisis management support during emergencies or urgent situations involving mental health crises or behavioral issues.
- Utilize motivational interviewing techniques to motivate clients toward positive behavioral change and improved well-being.
- Maintain detailed case records, write clear reports, and document all client interactions in compliance with agency policies and confidentiality standards.
- Offer bilingual support when necessary to serve culturally diverse communities effectively.
- Supervise or mentor junior staff or volunteers involved in client outreach or service delivery activities.
Skills
- Strong knowledge of social services systems including Medicaid, Medicare, Section 8 housing programs, child welfare, and workforce development initiatives.
- Proficiency in crisis intervention strategies and addiction counseling techniques.
- Experience working with seniors, individuals with disabilities, or geriatric patients is highly preferred.
- Excellent communication skills—both written and verbal—with the ability to effectively advocate for clients.
- Ability to perform behavioral health assessments and manage complex cases involving mental health or substance use issues.
- Skilled in resource identification within social services networks to connect clients with appropriate supports.
- Familiarity with CPR and First Aid certification; additional certifications in social work or counseling are advantageous.
- Bilingual abilities are highly desirable to enhance service delivery across diverse populations.
